About the Community Care Program
Fairview Red Wing Health Services’ community care program is part of our mission to provide quality medical services to everyone in our community. We treat all of our patients with dignity and respect, regardless of their ability to pay for our services. If you have concerns about your bill or cannot afford to pay, we want to help you.
If you are unable to pay all or part of your medical bill, do not have insurance or your insurance does not cover all your costs, you can apply for help through our community care program. Whether you are eligible for he community care program will depend on several things, including how much money you make.
We understand that there may be many reasons why you are unable to pay for medical care. Whatever the reason, you still need medical care. Fairview’s community care program can provide financial help for the treatment you receive.
Fairview Red Wing Health Services’ community care covers charges for most of our services. However, it does not cover charges for:
• Care that is not medically necessary (for example, cosmetic surgery, experimental treatments or care considered unnecessary by a Fairview doctor)
• Care that we do not offer at Fairview Red Wing
• Services given at Fairview Red Wing by independent contractors
• Doctor’s services not billed by Fairview Red Wing
• Services given to people who do not live in the United States
If you do not know whether the care you are seeing is covered by community care, please ask us.
Patients are expected to follow their insurance plan guidelines. Out-of-network services or ser services that would be covered by insurance at another facility may not be covered by community care.
Other health care providers may also have community care or charity care programs. Please contact those providers directly for non-Fairview-related services and bills.
To apply for community care…
1. You must first apply for help from the county and other community sources. Please call us at 651-267-5392 and we will help you with this process. If you choose not to apply for this help from the county, you may still qualify for some aid under our community care program.
2. If you are denied aid by the county, you must provide a written denial to Fairview Red Wing Health Services. If you choose not to apply for aid from the county or other community sources, you will have to show this on your application. Please remember that not applying for aid through the county will lower the amount of community care for which you qualify.
3. Get an application for our community care program. You can get an application by calling 651-267-5392.
4. Fill out the application form completely and attach copies of:
• Your written proof that you cannot get help elsewhere or are choosing not to apply for other aid,
• Your most recent federal income tax return and any schedules that go with it,
• And your pay stubs showing your income for the last 12 months.
5. Return the completed application to us within three weeks. We will let you know two weeks after receiving your completed application and written proof if you are eligible for community care.
