Finding services
A wide variety of services for older adults are available in the broader Red Wing service area. We can counsel you on senior housing options, transportation services, in-home services, palliative care, as well as end of life care. A Senior Patient Advocate knows and understands what is available—and knows how to connect you to the right service or program at the right time.
Personal Assessment
We start with a personal assessment to help understand the types of services you might need. Our health care professional will answer your questions, provide guidance, and will also suggest preventive measures to promote wellness.
Detailed Plan
The assessment leads to the development of a detailed plan of care to meet your specific needs. Your plan will include information about programs and services appropriate for you. When we agree on a plan, we will assign an “advocate” to help arrange and coordinate the plan implementation. Your advocate will also contact you on a regular basis to see how the plan is working and suggest any needed changes that could improve your quality of life. We can also work with your primary care provider to keep them informed about your health.
